Cause 2 of 8: You're asked to key in your PUK

If an incorrect PIN is entered three times in a row, you need to key in the PUK.

Solution: Key in the correct PUK.
Please note that if you haven't received a PUK or don't know it, it can be obtained from My Vodafone.
